ABSTRACT

BACKGROUND: The interrelationship of psychological and social factors in the current COVID-19 pandemic has been highlighted in research mainly focused on the global north. The impact of lockdowns can exacerbate psychological distress and affect access to services. Less is known about the psychosocial impact on women in the context of lower-middle income countries (LMICs); the aim of this study was to capture the impact of COVID-19 on women's experiences of pregnancy, birth and postpartum in Indonesia. METHODS: We conducted a rapid cross-sectional online survey of women across all 34 provinces in Indonesia to capture participants' experiences. Data were collected between 10th July to 9th August 2020 including demographics, effects on general and mental health and impact on service use. Descriptive statistics and thematic analysis were used to analyse responses, including those women who self-identified with a pre-existing mental health problem. RESULTS: Responses were obtained from 1137 women, this included pregnant women (n = 842) and postpartum women (n = 295). The majority of women (97%) had accessed antenatal care during their pregnancy, but 84% of women reporting feeling fearful and anxious about attending visits, resulting in some women not attending or changing provider. A small number (13%) were denied the presence of a birth companion, with 28% of women reporting that their babies had been removed at birth due to protocols or baby's health. Feeling anxious was a common experience among women (62%) during their pregnancy, birth or postnatal period, with a small number (9%) feeling depressed. Lockdown measures led to tensions within personal and family relationships. CONCLUSIONS: Women in Indonesia reported that the pandemic added an increased burden in pregnancy, birth and post-partum period: physically, psychologically, spiritually and financially. Maternity services were disrupted and health insurance cover lacked responsiveness, which either directly or indirectly impacted on women's choices, and equal access to care. Given the longevity of the current pandemic there is a need to develop tailored supportive interventions for women and their families and develop bespoke training for midwives and other relevant health professionals.

ABSTRACT

Selama prevalensi pandemi yang tinggi wanita hamil sebagai kelompok rentan yang terkena kerusakan psikologis yang tinggi. Pandemi COVID-19 dapat menimbulkan efek psikologis pada ibu hamil. Dibandingkan dengan sebelum wabah COVID-19. Kecemasan selama kehamilan juga meningkatkan risiko persalinan premature BBLR preeklamsia dan sesar. Mereview hasil penelitian terkait tentang tingkat kecemasan dan stress yang dirasakan ibu hamil selama masa pandemic COVID-19. Metode Scoping Review menggunakan framework Arskey & O’Malley. Saat merumuskan focus review dan straregi pencarian peneliti menggunakan format: Population Intervention Comparison Outcome (PICO) artikel yang direview sebanyak 10 artikel diperoleh dari 4 data base yaitu Pubmed Science direct dan Wiley. Seleksi artikel berdasarkan Prisma Flowcharts dan Critical Appraisal dengan Mixed Methods Appraisal Tool (MMAT). Berdasarkan artikel yang telah dipilih 10 artikel menggunakan metode kuantitatif 7 menggunakan study cross-sectional 2 menggunakan case-control 1 menggunakan survey kuantitatif. Didapatkan 4 sub tema yaitu: dampak factor penyebab tingkat kecemasan dan stress selama pandemi COVID-19 dan strategi penanganan. Perubahan psikologis terjadi pada ibu hamil selama masa pandemic COVID-19 ibu menunjukkan gejala kecemasan stress bahkan depresi. Factor yang mempengaruhi ialah: riwayat penyakit mental tingkat pendidikan kecukupan pendapatan rumah tangga dukungan pasangan dan kepuasan hidup perkawinan. Pada wanita hamil gejala kecemasan dapat meningkat lebih  karena kekhawatiran tentang kesehatan janin (merupakan penyebab stres yang dapat memicu indikator kecemasan) tetapi juga karena ketakutan tertular virus dan gangguan perawatan prenatal. Dalam keadaan social distancing hotline psikologis dan konseling online akan menjadi strategi yang aman dan layak untuk mengelola penyakit mental perinatal

ABSTRACT

Salah satu pelayanan kesehatan esensial yang tetap menjadi prioritas utama untuk dilaksanakan pada masa pandemi COVID-19 yang terjadi saat ini adalah pelayanan imunisasi bayi atau anak. Untuk melaksanakan prioritas tersebut, dilakukan beberapa tindakan penting dalam memastikan bayi atau anak yang merupakan kelompok paling rentan menderita PD3I dapat terlindungi dari penyakit-penyakit berbahaya. Tujuan penelitian adalah untuk mereview evidence mengenai implementasi pemberian imunisasi bayi pada masa pandemi COVID-19. Metode scoping review ini menggunakan framework Arksey & O'Malley yang terdiri dari 5 tahapan yaitu mengidentifikasi pertanyaan scoping review dengan framework PICO;mengidentifikasi artikel yang relevan dengan menentukan kriteria inklusi dan eksklusi;mencari literatur melalui database yang relevan, yaitu Pubmed, Science Direct, ProQuest dan EBSCO, sedangkan Grey Literatur diakses melalui Google Scholar;proses seleksi artikel ini peneliti menggunakan prisma flowchart untuk mendeskripsikan secara transparan bagaimana proses yang sudah dilakukan;melakukan Critical Appraisal untuk menilai kualitas artikel;melakukan data charting;menganalisa artikel, membuat mapping tema dan melakukan pembahasan pada artikel yang diperoleh. Berdasarkan 10 artikel yang diperoleh, didapatkan 3 artikel dengan grade A, 6 artikel grade B dan 1 artikel grade C. Selanjutnya didapatkan 4 tema yaitu Dampak pelayanan Imunisasi bayi, hambatan dalam pelayanan imunisasi bayi. harapan dan kebutuhan dalam pelayanan imunisasi bayi, dan upaya dalam pelayanan imunisasi bayi di masa pandemi COVID-19. Implementasi pemberian imunisasi bayi di beberapa Negara berkembang mengalami penurunan kunjungan namun ada dua daerah di Indonesia yang masih stabil. Sedangkan hasil penelitian di Negara maju (Amerika) juga mengalami penurunan kunjungan imunisasi selama pandemi COVID-19 ini. Kata kunci: Imunisasi Bayi;Implementasi;COVID-19;Pandemi One of the essential health services that remains a top priority to be implemented during the current COVID-19 pandemic is infant or child immunization services. To implement these priorities, several important actions were taken to ensure that babies or children who are the most vulnerable groups suffering from PD3I can be protected from dangerous diseases. This article aims to review evidence regarding the implementation of infant immunization during the COVID-19 pandemic. This scoping review method uses the Arksey & O'Malley framework which consists of 5 stages, namely identifying scoping review questions with the PICO framework;identify relevant articles by determining inclusion and exclusion criteria;search for literature through relevant databases, namely Pubmed, Science Direct, ProQuest and EBSCO, while Gray Literature is accessed through Google Scholar;In this article selection process, the researcher used a prism flow chart to describe transparently how the process had been carried out;perform a Critical Appraisal to assess the quality of the article;perform charting data;analyzing articles, making theme mapping and discussing the articles obtained. Results: Based on the 10 articles obtained, 3 articles were obtained with grade A, 6 articles in grade B and 1 article for grade C. Furthermore, 4 themes were obtained, namely the impact of infant immunization services, barriers to infant immunization services. hopes and needs in infant immunization services, and efforts in infant immunization services during the COVID-19 pandemic. The implementation of infant immunization in several developing countries has decreased visits, but there are two regions in Indonesia that are still stable. Meanwhile, research results in developed countries (America) also experienced a decrease in immunization visits during the COVID-19 pandemic.
